Somebody at home set the definition
Somebody in your household decided what counted as making something of yourself, and you have been either meeting or refusing that definition ever since. Pluto in Cancer in the 10th house ties your public standing to the family, so a career decision has never been only about work.

Write it down in one sentence and decide, on paper, whether you still endorse it, since it has been running unexamined for decades.
Success measured by whether they are safe
Your definition of having done well is whether the people you are responsible for are secure, rather than anything about your own position. That is coherent and it means you have never once measured your life on your own behalf. Ask what you would count as a good decade if nobody depended on you, and answer it seriously.
Earning as a form of protection
Money for you is what stands between the people you are responsible for and a bad year, which is why you cannot treat a professional risk as a personal one. Nobody around you knows that this is the calculation. Say it out loud to whoever is affected, since they may be willing to accept more risk than you have been assuming on their behalf.
You are unwell only when nobody needs you
Your body waits for a gap: the collapse arrives at the start of a holiday or the week after a crisis ends, never during it. That is not coincidence, it is a system that will not report while it is on duty. Build a genuine trough into the schedule before the exhaustion has to create one for you.
