Book recovery with the seriousness you book a deadline, and treat the appointment as fixed. Mountains are finished by climbers still functional at the top.

Your reliability makes you the person a job cannot afford to lose, which is exactly the argument for leaving you where you are. Nothing about doing the work well will move you. Set a date, prepare the case, and make the move, since the only mechanism that has ever advanced you is one you operated yourself.
Rest booked like a deliverable
You will not stop on the basis of feeling tired, because tiredness does not read as a reason. It has to be scheduled, dated and defended the way a commitment to somebody else would be. Put the days in at the start of the year, tell one person they exist, and treat moving them as the same kind of failure as missing a deadline.
State the ambition, since the work will not
Your record is excellent and entirely silent, and the people making decisions are working from impressions rather than from evidence they have gone looking for. Say plainly what you want next and by when, to the person who can act on it, once a year on a fixed date. Organisations move stated intentions considerably faster than they move somebody who appears content.
Working relationships that outlasted the jobs
You keep people across decades and changes of employer without much contact, because the regard was built on something that does not decay. That network is considerable and has essentially never been used. Ask three of them for something specific this year, since goodwill that is never tested quietly stops being available without anybody announcing it.
Duty took the place of choosing
A great deal of what you do is done because it should be done, and the question of what you would want has rarely been asked in those terms. That is a workable operating system with one flaw: it will run indefinitely without ever checking the destination. Ask the question annually, in writing, and be prepared for a dull answer.
Certainty is the product you are underpricing
What you actually supply is that nothing goes wrong, and it is being paid for as though it were hours in a chair. Nobody is going to volunteer the distinction between somebody who works hard and somebody whose work never has to be revisited. Put three examples of avoided failures on a page, attach a number to what each would have cost, and take it into the conversation.
